Quality Measures

Quality of Care

Joliet Area Community Hospice (JACH) has always been proud to provide quality care to all who come to us in need of the services we provide. One way to learn about a hospice’s quality is by reviewing their survey scores. Survey results also help us see areas where we are doing well and identify where improvements are needed.

Hospice Services

The Consumer Assessment of Healthcare Providers and Systems (CAHPS) Hospice Survey was designed to measure and assess the experiences of patients who died while receiving hospice care, as well as the experiences of their informal primary caregivers. The Centers for Medicare & Medicaid Services (CMS) developed the CAHPS Hospice Survey with input from many key stakeholders and this survey is the hospice industry standard.  Its focus is on the national priorities of improving care in the areas of involving patients and families in care and promoting effective communication and coordination.

The standardized 47-question CAHPS Hospice Survey measures the following: Communication with Family, Getting Timely Help, Treating Patient with Respect, Emotional and Spiritual Support, Help for Pain and Symptoms, Training Family to Care for Patient, Rating of this Hospice, and Willingness to Recommend this Hospice.

We are proud to report that JACH CAHPS scores exceed the state and national averages in all measures.

Hospice Programs are surveyed by the Illinois Department of Public Health every three years to determine quality and compliance with regulations. These surveys assess an organization’s performance in specific areas with requirements to ensure patient care and services are provided in a safe manner.  JACH was surveyed in January 2018 and received no deficiencies.

Joliet Area Community Hospice operates a free-standing Hospice Home inpatient unit where patients come for short stays to receive intensive symptom management. Inpatient units must be surveyed annually.  In January 2019, the Illinois Department of Public Health surveyed the inpatient unit at JACH and no deficiencies were cited.

We are proud of the quality care we provide as has been determined by these rigorous, unannounced surveys.

Grief Support Services

JACH offers a variety of grief support services to family members and anyone in the community dealing with the loss of a loved one. To best serve our clients, it’s important that we understand the effectiveness of our services.  At JACH, we measure and benchmark satisfaction with our services using a Bereavement Service Satisfaction Survey called Hospice Grief Support Services (HGSS) Survey. Adapted from National Hospice and Palliative Care Organization’s (NHPCO) Evaluation of Grief Support Services survey, our survey provides valuable insights from our clients about our bereavement services.  We are proud to report that our scores on this survey are above the national averages in all categories.

Our patients and families are our highest priority.  Please call (815) 740-4104 at any time, day or night, so we can assist you.
